Enrolling with Bloch Care CA
Choosing the right day program for your loved one is a big step, and we want to make the enrollment process as smooth as possible. Below is everything you need to get started.
1. Contact Your Case Manager
Before Bloch Care CA can begin providing services, please let your Regional Center Service Coordinator know that you’re interested.
2. Complete Our Onboarding Forms
Once your service coordinator is aware of your interest, please complete our onboarding documentation. These documents help us build a comprehensive understanding of your loved one’s needs and preferences, ensuring we can deliver person‑centered support from day one. These documents are also required by the Department of Developmental Services (DDS) for enrollment in our Adult Day Program, in addition to a position report, TB tests, a copy of identification, and an Individualized Program Plan (IPP).
